At the Northeast Kingdom Council on Aging, we help older Vermonters remain safely in their homes—on their terms. Whether you need help right away or just have questions, it all starts with a simple phone call.
We walk beside you every step of the way, offering trusted support through every stage of aging.
How It Works
- Call Our Helpline or Get Referred
Most services begin with a quick call to our Helpline at 800-642-5119, where our Information & Referral (I&R) Specialists are ready to help. You can also be referred by a doctor, hospital, or community partner. Our team will provide answers, guidance, and immediate connections to resources like housing, transportation, and more. - Options Counseling
If more support is needed, an Options Counselor will work with you to review choices and develop a short-term plan. - Case Management
When long-term support is appropriate, we assign a dedicated Case Manager. We are the only agency in our region authorized to provide Conflict-Free Case Management, meaning we serve only your best interests—never those of a provider network.
Our Independent Living Services Include:
- Personalized case management
- Help applying for benefits (Medicare, Medicaid, housing, fuel assistance, etc.)
- Coordination of services like meals, transportation, and in-home support
- Nutrition counseling from our staff dietitian
- Safety and accessibility planning

Options Counseling
Our trained Options Counselors work with you to identify the supports you need to continue living independently. Whether it’s help applying for 3SquaresVT or fuel assistance, navigating housing challenges, or beginning the Choices for Care application process—we’re here to help you understand your options and take the next step.
Long-Term Case Management
When ongoing support is needed, our Case Managers provide personalized guidance and coordination. We help you access services that allow you to stay safely at home—whether through state programs or local partners.
NEKCOA is the only agency in the region authorized to provide Conflict-Free Case Management, meaning we serve your needs—not those of a service provider.
We collaborate closely with local health and human service organizations, including NVRH, Northern Counties Health Care, North Country Hospital, and Caledonia Home Health & Hospice, to ensure you receive seamless care.
Care Transitions
Returning home from a hospital, rehab, or nursing home stay can feel overwhelming. Our staff will meet with you before discharge to assess your needs, coordinate with other providers, and help create a personalized care plan. We’ll support you throughout your transition and beyond—so you feel confident and supported at home.
