Office Hours & Appointment Information

The Northeast Kingdom Council on Aging is open Monday through Friday, 8:00 AM to 4:00 PM, with a lunch closure from 12:00 to 1:00 PM daily.


  • St. Johnsbury Office: Open for walk-ins for basic information or to schedule an appointment.
  • Newport Office: Open by appointment only.


To schedule an appointment at either location, call 800-642-5119 or email info@nekcouncil.org. If you reach us outside of regular hours, please leave a message—we’ll return your call as soon as possible.

At the Northeast Kingdom Council on Aging, our Information and Referral (I&R) Specialists are here to help you understand your options and connect with the right resources.

Transition Coordination

Transition Support You Can Count On

Navigating a move from a hospital, rehab center, or nursing home back home can be overwhelming. That’s why the Northeast Kingdom Council on Aging offers dedicated transition coordination to help make the process smoother and safer.


Our Transition Coordinator, Kim Rivard, brings over 13 years of experience at NEKCOA and is nationally certified in Information and Referral. She works closely with clients and families to ensure a successful return home—coordinating services, follow-up care, and support that meet each individual’s unique needs.


Kim partners with local hospitals, nursing homes, physicians’ offices, and service providers across the region to build strong, reliable support plans. Her deep knowledge of community resources and long-standing relationships in the field help remove barriers and make sure no one falls through the cracks.


This service is available at no cost and is designed to support health, independence, and peace of mind after a stay in a care facility.
