The Northeast Kingdom Council on Aging is open Monday through Friday, 8:00 AM to 4:00 PM, with a lunch closure from 12:00 to 1:00 PM daily.
St. Johnsbury Office:
Open for walk-ins for basic information or to schedule an appointment.
Newport Office:
Open by appointment only.
To schedule an appointment at either location, call 800-642-5119 or email info@nekcouncil.org. If you reach us outside of regular hours, please leave a message — we’ll return your call as soon as possible.

At the Northeast Kingdom Council on Aging, our Information and Referral (I&R) Specialists are here to help you understand your options and connect with the right resources.
We can help with:
- Personalized information and referrals for local services
- One-on-one options counseling to explore care, housing, and benefits
- Help with transitions after a hospital, rehab, or nursing facility stay
- Medicare, Medicaid, and health insurance guidance through SHIP
- Support for caregivers, including respite and local resources
- Referrals for legal assistance such as wills and advance directives
- Navigation of long-term care services in the home, including Choices for Care
- Connections to elder rights advocacy and the Long-Term Care Ombudsman
- Enrollment in home-delivered meals or access to community meal sites
- Information on transportation options and ride coordination
- Access to evidence-based health and wellness programs
- Referrals for home modifications or assistive equipment
Sometimes your questions can be answered in a quick phone call. Other times, we may walk you through online tools to check your eligibility for state and federal programs.
For more complex needs, we can refer you to an Options Counselor for a personalized consultation — always free of charge.

Transition Coordination
Transition Support You Can Count On
Navigating a move from a hospital, rehab center, or nursing home back home can be overwhelming. That’s why the Northeast Kingdom Council on Aging offers dedicated transition coordination to help make the process smoother and safer.
Kim partners with local hospitals, nursing homes, physicians’ offices, and service providers across the region to build strong, reliable support plans. Her deep knowledge of community resources and long-standing relationships in the field help remove barriers and make sure no one falls through the cracks.
